Treatment of Uncomplicated Male Gonococcal Urethritis with Clavulanate-potentiated Amoxycillin(Singl does vs 3 day regimen)

Abstract
In previous report, we estimated the effectiveness of single dose and 5 day regimens with clavulanate-potentiated amoxycillin for uncomplicated male gonococcal urethritis. Since all patients treated with 5 day regimen had recovered, we are compelled to study a regimen of shorter duration, 3 days. From January to March 1987, 140 bacteriologically proven male uncomplicated gonococal urethritis patients at the VD clinic of Choong-Ku Public Health Center were allocated randomly into one of the 2 regimens to be compared. Sixty-seven of the 69 patients treated with clavulanate-potentiated amoxycillin, 3.25g, PO plus probenecid, 1g, PO were followed. Four(6.0%) failed to be cured. Sixty-three of the 71 patients treated with clavulanate-potentiated amoxoycillin, 375mg, PO, t. i. d. for 3 days were followed. One(1.6%) failed to be cured. There is no significant difference between the 2 groups in failure rates(p>0.05). In PPNG urethritis the failure rates were 12.1%(4/33) and 0%(0/30) in single dose regimen group and 3 day regimen group respectively. There is a significant difference in failure rates between the two groups(p<0.05). It is concluded that 3 day regimen of clavulanate-potentiated amoxycillin has excellent effect in the treatment of goncoccal urethritis. Because of high rate of Penicillinase Producing Neisseria gonorrhoeae among circulating N. gonorrhoeae, the single dose regimen is only marginally effective for uncomplicated gonococcal urethritis in men in Korea.
